Set in the distant future, Dead Space revolves around Isaac Clarke, a space engineer who works for the Concordance Extraction Corporation (C.E.C.), a company that operates giant mining ships throughout the galaxy. When the C.E.C. receives a distress call from the USG Ishimura (a “Planet Cracker”-class ship that destroys planets in order to extract valuable ore), Isaac and four other C.E.C. employees set out on the USG Kellion to rendezvous with the Ishimura in orbit above Aegis-7. Their assumption is that a mechanical failure is the only problem. At the beginning of the game, Isaac is seen repeatedly watching a video sent to him by his girlfriend, Nicole Brennan, who is currently onboard the Ishimura.

Shortly after crash landing on the Ishimura flight deck, the crew of USG Kellion is attacked by hostile creatures called Necromorphs. These creatures reproduce by infecting deceased human bodies, violently mutating them into one of their own. Isaac escapes, but is separated from the rest of the team. At this point, Isaac, Kendra Daniels, and Zach Hammond are the only surviving members. As Isaac progresses through the Ishimura and fixes the various problems onboard, he finds Nicole alive and well, though she behaving strangely, and repeatedly asking him to "make [them] whole again". Kendra and Hammond discover the main cause of the outbreak is due to a supposedly ancient, alien artifact found on Aegis-7 called the Marker, which is onboard the Ishimura. Kendra is able to send out an emergency signal to the USG Valor, a military ship, for help.

Unfortunately, the Valor picks up an escape pod containing a Necromorph, earlier trapped by Hammond, who launched it into space. The Valor crashes into the Ishimura and Isaac manages to get onboard and grab the ship’s core, which will be used on the Ishimura’s only possible escape vessel. Hammond shows up for support, but is quickly killed by a Necromorph. As Isaac escapes from the exploding ship, he runs into Dr. Terrence Kyne, who reveals the existence of the Hive Mind, the controlling force of the Necromorphs. Kyne pleads with Isaac to help him return the Marker to the planet. Kyne reveals that the Marker had contained the Hive Mind, but was freed when the artifact was removed and Aegis-7 "cracked open".

Before Isaac can rendezvous with Kyne, Kendra kills the doctor, revealing herself as a covert agent and betrays Isaac. She explains that the marker on Aegis 7 is not a genuine alien artifact but merely an imitation of the original marker which had been discovered on Earth many years before. This marker was made for the express purpose of scientific experiments, and replicating the original alien technology which resulted in the creation of the Necromorphs. After she reveals this information Kendra escapes, taking the Marker with her. Fortunately, Isaac meets up with Nicole and they override the shuttle Kendra escapes on. The two use the vehicle to return the Marker to the Aegis-7 and after a lengthy delivery, they finally set the Marker down and expect everything to be returned to normal. However, Kendra appears and informs Isaac that Nicole has been dead throughout the events on the Ishimura, and his meetings with her were never real, but an illusion the marker used to manipulate Isaac. As proof, she shows Isaac the same video he was watching before their arrival on the Ishimura, but asks that he watch the whole thing. Isaac then sees that Nicole had committed suicide at the end of her transmission. In a last-ditch attempt to destroy the Necromorphs, Kendra releases the gravity tethers anchoring a large asteroid above the planet which begins its fiery descent. Isaac dashes after Kendra, who has already made it to the ramp of the shuttle, but before she can enter the ship a huge tentacle from the Hive Mind smashes down upon her and kills her. Isaac is left to defeat the Hive Mind of the Necromorphs and escape the doomed planet on his own. Shortly after the planet explodes, Isaac is finally relaxing after his quick escape when the video of Nicole’s last transmission pops up on display. He turns the video off just before hearing a quick noise in the cabin of the craft. As he investigates this, he notices a presence crouching in the dark with the looks of Nicole just as it leaps to attack.
